The Health Office is staffed by our nurse, Mary Ann Sweeney RN from 8:30 AM to 2:30 PM Monday through Friday. The nurse can be reached by phone at 201-327-1108 . 1. Admission to school: a. The student’s immunization record with documentation of state mandated immunizations must be presented to school before admission to school. b. Each student must have a physical examination by his/her healthcare provider within 365 days prior to entry to school. The health care provider must complete Athletic Pre-Participation and Physical Evaluation Form Part B. *Form Part B may be downloaded from this site or can be obtained from the school nurse. c. Personal and Family Health History Form completed and signed by parent for guardian. d. A complete eye examination by an Ophthalmologist or Optometrist is recommended. e. A dental examination is recommended. 2. Athletic Pre-participation Annual requirements: a. Athletic Pre-participation Form Part A* must be completed and signed by parent or guardian within 365 days prior to first practice of the sport. b. Athletic Pre-participation Form Part B* must be completed by the student’s health care provider based on a physical examination done within 365 days prior to the first practice of the sport. *Forms Part A and B may be downloaded from this site below or obtained from the school nurse.
a. It is recommended 3rd and 6th grade students have a well physical examination by a health care provider utilizing Athletic Pre-participation and Physical Examination Form B.* b. A Personal and Family Health History Form updated by the parent/guardian is requested. The Health History Form* can be downloaded from this site or obtained from the school nurse. Policy on Administration of Medication St. Paul Interparochial School strongly discourages the administration of either prescription or non-prescription medication in school or on field trips. However, if it is absolutely essential that a student receives medication while under school supervision, the following procedures apply: 1. A parent/guardian should come to the school and personally administer the medication. 2. If this arrangement is not possible, the school nurse or principal (or his/her designee) will administer the medication under the following conditions: A. The medication must be given to the school nurse or principal by the parent/guardian B. The medication must be in the original pharmacy-labeled container. C. A parent/guardian and the student's physician must complete and sign an "Authorization to Administer Medication in School" form, available from the school office. Students will be permitted to self-administer medication only for life-threatening illnesses or conditions. Parents/guardian must schedule a conference with the school nurse to discuss in detail the need for medication.
